Where are you trying to cast the spell?
Your post says you're trying in the Temple of Chaos, which is wrong. The
body of the Hierophant of Chaos is in the courtyard of the Temple of
Enthusiasm. You usually can get the spell to work from the Serpent Gate.

That should work.

In the basic game this was accidently left out. The actual coding and
dialogues were done, but not actually switched on in the final version.
Some of the Exult upgrades fix the issue so you can cure her.

Nothing official as in an option built into the original game itself, but
you might try Lord Empire Dragon's cheats at the
Backpacker's Guide to Britannia. http://members.shaw.ca/ultima/index.htm

My own advice is to first save the game and then steal like hell from
the weapons and armour vendors, reloading if needed. You can pick up the
best weapons and armour quickly even without a thief character, which
makes the rest of the game much easier.

No. The speakings of cured Cantra are in the usecode, but this plot path was
not coded, it was only started.

Have you tried running the old California Pacific release in an Apple
emulator? To me it feels like the opponents don't do as much damage in
that version. Though to be honest I didn't read the code so maybe this
is just an illusion.
